REGISTRATION PROTOCOLS

Cost

• This event will accommodate a limited number of attendees. To guarantee your spot you must pay a non-refundable fee of $50.00 USD. This amount will be refunded at the Forum together with the reimbursement check for the airfare (in U.S dollars).
• KFLA will cover the costs for hotel (double room occupancy), meals (4 breakfasts, 3 lunches, 3 dinners) and a % of an economy cabin service airfare. If you travel by car or bus, you will need to cover your own travel expenses.

Airfare

• You must pay 100% up front for your airfare ticket. KFLA will reimburse you at the Forum according to the reimbursement system described here. If you book a flight with another type of cabin service (Business, First Class, etc), KFLA‘s percentage reimbursement will be based upon the standard price for an economy class airfare ticket from the country you are traveling from.
• You must submit your airfare booking confirmation by August 8, 2014. It must indicate the amount you paid and your arrival and departure times. Please send your travel confirmation (e-ticket or PDF), to raragon@kfla.org.
• You may purchase your airline ticket through the U.S. travel agent, “On The Mark Travel”. This option is only available if you purchase your ticket with a credit card. On The Mark Travel will credit your card on the final day of the event, Sunday September 7th, for any reimbursement that is owed to you. Contact information follows:

Reimbursement

• KFLA will reimburse economy class airfare costs after the first $550USD (i.e. you are responsible for the first $550USD and KFLA will reimburse the rest) up to a maximum reimbursement amount of $1000USD
• Airfare booking confirmation must be sent no later than August 8, 2014 (08/08/14).

You are eligible for partial reimbursement of your economy-class airfare subject to the following conditions:

• You must purchase your airfare and submit it to KFLA by August 8, 2014
• Your airfare price must not be greater than the 21-day advance price
• You MUST attend Foro 2014 in its entirety to be eligible for reimbursement. You MUST arrive by the Welcome Dinner on Sept 6th at 6:00PM and leave after 1:00PM on September 7th. To accommodate customs and ground transportation, your inbound flight should arrive in Panama no later than 3PM and your outbound flight should depart no earlier than 1PM
• All reimbursements will be given in USD on Sunday Sept 7th after the event concludes. Reimbursement is only available by the following methods: PayPal, Western Union Money Order, Check in USD from a US Bank

The following are NOT eligible for reimbursement:

• First-class or other premium-cabin service or upgrades. KFLA will only reimburse up to the value of an equivalent economy-class airfare on the same route
• Airfare booked after August 8, 2014 and/or exceeding the 21-day advance price
• Cab fares, private transportation, or other ground transportation to/from Tocumen International Airport and venues other than the round-trip shuttle service provided by KFLA as part of your registration.
• Any and all ground transportation in your country of origin and/or to/from your airport of origin
• Additional costs you may incur due to schedule changes or missed flights
• Visa costs, other administrative fees, or other travel costs

Accommodation

• The rooms will have double occupancy as an opportunity for fellows to network. If attendee is interested in a room with single occupancy, KFLA will not be responsible for payment of an individual room.

Transportation

• KFLA will provide round trip shuttle service between Tocumen International Airport and the hotels. KFLA will not reimburse for cab fares or other private transportation services to/from the airport or any other location.

Visa

• The citizens of the following countries require a stamped visa to enter Panama: Ecuador, Haiti, Peru, and Dominican Republic. Contact your local embassy or consulate for the most up-to-date information or visa forms.

Insurance

• You are responsible for buying your own personal health and evacuation insurance.

Tourism

• Attendees are invited to visit touristic sites at their own expenses before or after the Forum. Attendees are expected to be present at all event sessions.

Guests

• This is a Fellows-Only event. Family and friends may join you after the event.

Registration

• Registrations will close at midnight EST on August 8, 2014 (08/08/2014) or once we have reached the magic number of 100 attendees. Registrations made after that will be added to a wait list.

Liability Waiver

Kellogg Fellows Leadership Alliance, Inc. ( KFLA) and its affiliates, including On the Mark Travel, serve only to assist in making necessary travel arrangements for Kellogg Fellows/Advisors and their travel partners (from here on referred to as “participants”), and in no way represent, or act as agent for transportation carriers, hotels, and other suppliers of services connected with this trip. KFLA and/or its affiliates are not liable for any injury, damage, loss, accident, delay or other irregularity that may be caused by the defect of any vehicle or the negligence or default of any company or person engaged in performing any of the services involved. Additionally, responsibility is not accepted for losses or expenses due to sickness, weather, strike, hostilities, wars, epidemics, pandemics, natural disasters or other such causes.

KFLA and/or its affiliates do not accept liability for any airline cancellation or delay incurred by the purchase of an airline ticket. Baggage and personal effects are the sole responsibility of the owners at all times. KFLA and/or its affiliates reserve the right to make changes in the published itineraries whenever, in their judgment, conditions so warrant, or if it is deemed necessary for the comfort, convenience or safety of the participants. KFLA and/or its affiliates also reserve the right to decline to accept any person as a participant, or to require any participant to withdraw from the program at any time, when such an action is determined by the appropriate staff representative to be in the best interests of the health, safety, and general welfare of the group, or of the individual participant.

The participant accepts any risks thereof and the conditions set forth therein. The participant also understands and hereby agrees for and on behalf of himself/herself, his/her dependents, heirs, executors, administrators, and assigns to abide by the conditions set forth under LIABILITY WAIVER, above, and to release and hold harmless Kellogg Fellows Leadership Alliance and/or its affiliates and any of their officers or representatives from any and all liability for delays, injuries or death, or for the loss of or damage to his/her property however occurring during any portion of the program.

Cancellation Policy

Should a situation arise which prevents you from attending Forum, you may transfer your registration to another participant, or you may donate your registration fee to KFLA. However; because of KFLA financial commitments, the registration fee is non-refundable after registration has been submitted. No part of the registration fee will be refunded under any circumstances (e.g. weather, medical, business
conflict, medical emergency, etc.). Participants desiring protection against potential emergencies should consider purchasing travel insurance.

Photo and Video Release

For valuable consideration received, I hereby grant to KFLA (photographers and videographers) the absolute and irrevocable right and unrestricted permission in respect to photographic portraits or pictures taken of me or in which I may be included with others, to use, reuse, publish, and republish the same in whole or in part, individually or in any and all media now or hereafter, and for any purpose whatsoever, for illustration, art, editorial, advertising, and trade, or any other purpose whatsoever without restriction as to alteration; and to use my name in connection therewith if he/she so chooses.

I hereby release and discharge Photographer and Videographer from any and all claims and demands arising out of or in connection with the use of photographs and videos, including without limitation any and all claims for libel or violation of any right of publicity or privacy.

This authorization and release shall also inure to the benefit of the heirs, legal representatives, licensees, and assigns of Photographer and Videographer, as well as the person(s) for whom he/she took the photographs and videos.

I am of full age and have the right to contract in my own name. I have read the foregoing and fully understand the contents thereof. This release shall be binding upon me and my heirs, legal representatives, and assigns.
